Thank
you for selecting Arizona Bone and Joint Specialists to serve your orthopaedic needs. It
is our intention to provide you with the best orthopaedic care available. This page
contains information to familiarize you with our practice and is designed to answer
questions that patients most often ask. Feel free to address further questions to any of
our staff. We are here to serve your needs. Our
practice is limited to the specialty of orthopaedics, which is devoted to the diagnosis
and treatment of disorders and injuries of the musculoskeletal system in both adults and
children. Sports medicine concerns the evaluation and treatment of injuries incurred in
the course of athletic endeavors. Our doctors are board certified orthopaedic surgeons. Each is
committed to providing you with the most current methods of treatment available. Participating in continuing education courses and study keeps us at the forefront of
orthopaedic progress. It is your option to select your personal
physician from among the doctors in our group. He will manage the great majority of your
care. Should you have a problem that requires immediate attention and your doctor is not
available, one of our other doctors will be available to assist you. Your doctor will
normally assume your care upon his return. If, in the case of an emergency, you are
taken to a local emergency facility, you may request that we be called to care for your
orthopaedic needs. We are always interested in
any suggestions as to how we can better serve the needs of our patients. Please contact
the office administrator at (602) 493-9361 with your concerns and comments.

Fees We share your concern about the increasing costs of
medical care. Because statements and billing fees have become so expensive, and in an
effort to keep your medical costs down, WE ASK THAT YOU PAY FOR OFFICE VISITS AT THE TIME
THE SERVICE IS RENDERED. Our fees are usual and customary for this area. Fees for surgery
include the usual follow-up care, but do not include x-rays, casts or bandages. The fee
for a first office visit or a new problem will be higher than the fee for a follow-up
visit. We will be happy to discuss fees with you, and an estimate of fees for any
surgical procedure will be given when requested. Co-Pays must be paid at the time of
check-in for an appointment.

Insurance and Medicare We remind you that THE RESPONSIBILITY FOR PAYMENT
RESTS WITH THE PATIENT BEING TREATED or the parent/guardian. For office visits we expect
payment at the time of that office visit by cash, check, or credit card
(Visa/MasterCard/Discover). If you have insurance coverage, we will submit the necessary
insurance forms to your insurance carrier. We remind you that most insurance contracts,
including Medicare, involve deductibles and/or percentage allowances with the result that
the entire bill is seldom covered in full. Should we receive any payment which exceeds
your balance due, the excess will be refunded to you. Feel free to address any questions
regarding your bill to our business office at (602) 493-9361 during regular office hours.

workman's compensation, you must bring, from your employer or insurance company, a
written acceptance of their financial responsibility. In liability or contested workman's compensation acts,
we remind you THE RESPONSIBILITY FOR PAYMENT RESTS WITH THE INDIVIDUAL BEING TREATED, not
necessarily the individual being sued. We expect you to pay your bill promptly. Once this
is done, we will fulfill our responsibilities in providing your attorneys with necessary
medical information. HMO patients must have a referral at the time of their visit. Obtaining the referral is
the patient's responsibility.
